Methods of Testosterone Replacement:

  1. Injections: Administered through intramuscular or subcutaneous injections, this method offers flexible dosing schedules, typically every one to two weeks. Injections provide rapid absorption of testosterone but may require regular visits to a healthcare provider.

  2. Topicals: Topical gels and creams are another convenient option, applied to the skin once daily. They allow for quick absorption and maintain stable hormone levels. Care must be taken to prevent transfer to others through skin contact.

  3. Pellets: Subcutaneous testosterone pellets, which are inserted beneath the skin, release testosterone gradually over several months. This method requires a small skin incision but offers a long-lasting solution without the need for daily attention.

Choosing the appropriate method of testosterone replacement therapy should involve thorough consultation with a healthcare provider to ensure safety and effectiveness tailored to individual health profiles and lifestyles.

How Does Testosterone Injection Therapy Work?

Testosterone injections work by supplementing the body’s natural testosterone production. When administered, the synthetic testosterone is absorbed into the bloodstream and binds to androgen receptors in various tissues, promoting the development and maintenance of male characteristics, energy levels, and overall health.

Benefits of Testosterone Injection Therapy:

  • Improved Libido: Enhanced sexual desire and performance.

  • Increased Energy Levels: Reduction in fatigue and increased vitality.

  • Mood Stabilization: Reduced irritability and depression, improved sense of well-being.

  • Muscle and Bone Strength: Increased muscle mass and strength, improved bone density.

  • Cognitive Function: Potential improvement in memory and cognitive abilities.

Potential Side Effects and Risks:

While testosterone injection therapy can be highly beneficial, it’s important to be aware of potential side effects. Some side effects are more common depending on the method of TRT delivery you choose. Overall, potential side effects of treatment include:

  • Site Reactions: Pain, swelling, or irritation at the injection site if taking weekly shots. Also risk of pellet extrusion (pellet erupting back to or thru the skin surface).

  • Hormonal Imbalances: Fluctuations in testosterone levels can cause mood swings or irritability.

  • Increased Red Blood Cell Count: This can lead to a higher risk of blood clots.

  • Gynecomastia: The development of breast tissue in men, due to an imbalance between testosterone and estrogen.

  • Prostate Health: There is some concern about testosterone therapy affecting prostate health, so regular monitoring is essential.

  • Cardiovascular Events: Studies have shown mixed results, with some indicating an increased risk of heart-related issues while others suggest minimal or no significant effect.

    Monitoring and Follow-Up:

Regular follow-up with a healthcare provider is crucial to ensure the therapy is working effectively and to monitor for any side effects. This typically includes:

  • Blood Tests: To monitor testosterone levels, red blood cell count, and other relevant markers.

  • Symptom Review: Discussing any ongoing symptoms or concerns with your healthcare provider.

Who Should Avoid Testosterone Injection Therapy?

Testosterone replacement therapy (TRT) can benefit those with low testosterone, but it is not suitable for everyone. Individuals who should avoid TRT include:

  • Prostate Cancer Patients: Increased testosterone may stimulate cancer growth.

  • Men with Untreated Sleep Apnea: TRT can worsen this condition, posing health risks.

  • Patients with Uncontrolled Heart Conditions: TRT may increase cardiovascular risks in unstable situations.

  • Individuals with Erythrocytosis: Elevated red blood cell counts could rise further with TRT, increasing clot risks.

  • Men with Active Liver Disease: Current liver issues complicate hormone therapy.

  • Those with Hormonal Disorders: Hormonal imbalances unrelated to low testosterone should be addressed first.

  • Minors: Not recommended for individuals under 18 due to impacts on growth.

Consult a healthcare provider before starting TRT to evaluate medical history and treatment risks.

Bioidentical Hormone Pellet Therapy

Men's hormone pellet therapy typically costs between $550 and $700 every 4 to 6 months. This treatment is a ‘one and done’ option for men who want to avoid frequent injections or not have to mess with topical creams and gels. These hormones are bio identical, so they carry less risk than the traditional methods of replacement (injections, creams/gels). The frequency and cost of therapy can vary based on individual needs and can be discussed at your appointment to find out which option is right for you. Additionally, you will be started on an oral supplement that you take twice daily to help control estrogen levels. This supplement is $50 a month and can be purchased at the time of your pellet insertion.
